Online Liability Waiver
Click the link below to fill out our online liability waiver. You will need to have this one time only waiver filled out in total before participating in any of our classes. Be prepared to show your confirmation e-mail to us when you show up to your 1st class at the studio so we can verify you. If you have trouble filling this out at home, you can always show up a little early for your 1st class and we will assist you with it in studio.
